To secure a spot in the 2026 Wooster Soccer Camp, you need to send in a registration form and a deposit or payment in full.  You can either email or use regular snail mail to send in your registration.

Make sure you list an email address on the registration form.  This is how you will be contacted and sent additional information!

Please give an email address that you check often. 

I always respond / confirm within 48 hours.  Some email programs (ie. yahoo) do not like my emails and maybe put in your junk folder. 

Email Registration

1.   Click on the “Registration Form” below (excel file), fill it out, save it as an Excel file (or     Numbers - for MAC users), and email it as an attachment .  In many PC’s, when you are in Excel, after you fill out the form, drop down from “File” and you can send mail directly.

DO NOT fill out the form if you are in your internet browser.  You must open the form on your desk top.  Drag it to your desk top and then fill it out

2. 2. Click on the “Waiver Form” below, print it out, and sign it.  Scan and email in
   

3. 3. A $200 deposit or payment in full needs to be sent in.  The waiver form will help you figure out your fee (especially if you have applicable discounts).  Failure to submit payment within 1 week may result in your registration being cancelled.

You will always receive email confirmations within 48 hours when any registration step is received and when the payment/deposit is received.  If you don’t receive a confirmation, please call or email to double check.

                                       

Mail-in Registration

1.   Please print out the registration form & waiver form above,  fill them out completely, and sign it.    

2.   Mail these in with your payment.    

Once the forms are received, you will receive a confirmation & additional information via email.

Medical Form

All campers must be in good physical condition to participate in the rigors of soccer camp.    Camper who are sick are not allowed to attend and must cancel their registration.  Please contact us prior to camp, if you have questions or concerns about your daughter’s participation in camp.  Campers must be able to administer their own medication, including Epipens. 

Each camper must have their own medical insurance coverage.  This camp medical form must be filled out and emailed in a clear black and white document prior to camp starting.  A parent signature is required on this form.  Note - This Medical Form is different from the Waiver Form.

Camp Rules

Camp rules are established to provide a safe and enjoyable camp experience for all.  Please make sure you print out these camp rules for your daughter and read through them with her.  All campers are expected to be emotionally ready for an independent camp experience.  Campers should not have discipline issues.  Breaking camp rules may result in the camper being sent home early.
